	A Display First, you'll need a 3D-ready display--whether it's a 3D HDTV, 3D projector, or 3D computer monitor. These displays have more processing power than standard 2D models for displaying 3D images in rapid succession.

การผ่าตัดเพื่อแก้ไขความผิดปกติของขากรรไกร เรียกว่า Orthognatic Surgery เป็นการผ่าตัดเพื่อปรับตำแหน่งของขากรรไกรร่วมกับการจัดฟัน เพื่อช่วยแก้ไขและปรับปรุงโครงหน้าที่ปกติ รวม ถึงการแก้ปัญหาการสบฟันให้มีความถูกต้องและเหมาะสม เนื่องจากในบางกรณีความผิดปกติของขนาด รูปร่างหรือตำแหน่งของกระดูกขากรรไกรเพียงเล็กน้อยนั้นสามารถส่งผลกระทบทำ ให้การสบฟันและการบดเคี้ยวผิดปกติ รวมไปถึงความผิดปกติของรูปหน้าอีกด้วย

ขั้นตอนการรักษา

การตรวจวินิจฉัยและการวางแผนการรักษา ทางเลือกในการรักษามีทั้งกรณีจัดฟันก่อน
ผ่าตัด และผ่าตัดก่อนจัดฟัน

การวางแผนการรักษาล่วงหน้าเป็นสิ่งสำคัญ และ เป็นปัจจัยสำคัญที่จะช่วยให้การรักษาประสบความสำเร็จนอกเหนือจากขั้นตอนการ รักษาและผ่าตัด โดยพิจารณาจากรูปภาพ แบบจำลองฟัน และภาพถ่ายรังสี

กรณีจัดฟันก่อนผ่าตัด เพื่อ ให้ได้ฟันที่มีการเรียงตัวและการสบฟันที่เหมาะสม จะช่วยให้เกิดการสบฟันที่ถูกต้องหลังเข้ารับการผ่าตัด และการจัดฟันหลังการผ่าตัดยังคงมีความจำเป็นในทุกกรณีเพื่อแก้ไขการสบฟันที่ อาจยังไม่ถูกต้อง สมบูรณ์ หรือเพื่อคงสภาพฟันหลังการผ่าตัด

การจัดฟันก่อนการผ่าตัดใช้เวลาประมาณ 1 - 2 ปี อาจ ต้องมีการถอนฟันร่วมด้วย หากฟันซ้อนเกมาก การจัดฟันก่อนผ่าตัดจะใช้เครื่องมือจัดฟันแบบติดแน่นเคลื่อนฟันในแต่ละขา กรรไกรไปในตำแหน่งที่สัมพันธ์กับขากรรไกรนั้นๆ เนื่องจากการสบฟันที่ผิดปกตินั้น ฟันจะล้มเอนตามธรรมชาติไปในแนวที่ปิดบังความผิดปกติของขากรรไกร ดังนั้นการแก้การล้มเอนตามธรรมชาติและจัดเรียงฟันให้ได้ตำแหน่งที่ถูกต้องบน แต่ละขากรรไกรก่อนการผ่าตัด มักจะทำให้การสบฟันก่อนผ่าตัดรวมทั้งใบหน้าดูแย่ลงชั่วคราว

เมื่อพร้อมที่จะทำการผ่าตัด จะ พิมพ์ปากและเอ็กซ์เรย์อีกครั้งเพื่อจำลองการผ่าตัดในแบบฟัน และเตรียมทำเฝือกสบฟันสำหรับใช้ในห้องผ่าตัด ผู้ป่วยจะได้รับคำแนะนำในการเตรียมตัวก่อนและหลังการผ่าตัด การผ่าตัดจะกระทำใน
โรงพยาบาล และผู้ป่วยมักจะต้องพักฟื้น 1-3 วันในโรงพยาบาล และพักต่อที่บ้านอีกประมาณ 2-4 สัปดาห์

การจัดฟันหลังการผ่าตัด เป็นการจัดฟันในรายละเอียดเพื่อให้การสบฟันสมบูรณ์ยิ่งขึ้น โดยมากจะใช้หนังยางดึงฟันร่วมด้วย และมักจะใช้เวลาประมาณ 6 เดือน

อาการที่อาจเกิดขึ้นได้ภายหลังการรักษา

ผุ้ป่วยจะใช้เวลาพักฟื้นที่โรงพยาบาลประมาณ 1 – 3 วันหลังการผ่าตัด ซึ่ง จะแตกต่างกันในแต่ละบุคคล ภายในสัปดาห์แรกหลังการผ่าตัดผู้ป่วยอาจมีอาการบวมหรือฟกช้ำบนใบหน้าซึ่ง เป็นผลจากการผ่าตัดและผู้ป่วยยังจำเป็นต้องได้รับการจำกัดและควบคุมด้าน กิจกรรมที่ต้องออกแรงมากๆ เช่น การออกกำลังกายภายในหนึ่งเดือนแรกหลังการผ่าตัด

การผ่าตัดเพื่อแก้ไขความผิดปกติของขากรรไกรนั้น ส่วน ใหญ่แล้วจะต้องรอให้ผู้เข้ารับบริการมีฟันแท้ขึ้นมาจนครบเสียก่อน นอกจากในบางกรณีเท่านั้นที่มีความจำเป็นพิเศษ เช่นกรณีอุบัติเหตุ ศัลยแพทย์จึงจะทำการผ่าตัดให้ทันที และการผ่าตัดจัดแต่งขากรรไกรในผู้ใหญ่นั้น บางครั้งยังสามารถเป็นศัลยกรรมเพื่อความงามโดยทำร่วมกับการผ่าตัดเนื่อเยื่อ บางส่วนเพื่อให้ได้รูปหน้าที่มีความสวยงามมากขึ้นได้อีกด้วย

โรคเหงือกอักเสบ

โรคเหงือกอักเสบเป็นปฏิกิริยาที่ร่างกายตอบสนองต่อเชื้อโรคที่สะสมรอบๆ ตัวฟัน พบ ว่าเกือบทุกคนจะเป็นโรคเหงือกอักเสบ อาจมากบ้างน้อยบ้างแตกต่างกันไป โดยปกติเหงือกจะทำหน้าที่ห่อหุ้ม และป้องกันอันตรายให้แก่กระดูกและฟัน โดยมีเอ็นยึดกับรากฟันเอาไว้ ขอบเหงือกที่บริเวณคอฟันจะมีลักษณะบาง ผิวเรียบ ไม่ยึดกับตัวฟัน ทำให้เกิดเป็นร่องเหงือกลึกประมาณ 2-4 มิลลิเมตร ส่วนของเหงือกที่อยู่ต่ำลงมาจะยึดกับกระดูกเบ้าฟัน ส่วนของเหงือกยึดนี้มีความแข็งแรงกว่าเหงือกที่อยู่บริเวณขอบเหงือก และถ้าสังเกตให้ดีจะเห็นว่ามีลักษณะขรุขระเล็กน้อย ไม่เรียบเหมือนขอบเหงือก

ประสิทธิภาพการทำหน้าที่ของฟันขึ้นอยู่กับความแข็งแรงของตัวฟันและอวัยวะที่อยู่รอบๆ ซึ่งทำหน้าที่ยึดฟันและพยุงฟันให้คงอยู่ในขากรรไกร อวัยวะที่อยู่รอบๆ ตัวฟัน มีชื่อเรียกรวมกันว่าอวัยวะปริทันต์ ซึ่งประกอบด้วย เหงือก เอ็นยึดปริทันต์ เคลือบรากฟัน และกระดูกเบ้าฟัน

ถ้าสังเกตดูที่เหงือกของเราจะเห็นว่ามีสีชมพูและซีดแต่ถ้าเกิดโรคมีการอักเสบหรือมีการติดเชื้อจะมีสีแดง เรียบเป็นมันหรือเป็นสีม่วง คล้ำ ช้ำ เมื่อ เหงือกมีการอักเสบและติดเชื้อจะมีหนองเกิดขึ้น โดยเฉพาะบริเวณขอบเหงือก ถ้าลองกดดูจะเห็นว่ามีหนองไหลออกมา อย่างไรก็ตามในบางคนอาจจะมีเหงือกสีคล้ำได้เช่นคนผิวดำ คนที่สูบบุหรี่เป็นประจำนานๆ หรืออาจจะพบได้ในคนที่ทำงานสัมผัสกับสารเคมีเป็นประจำนานๆ

สาเหตุ

เชื้อแบคทีเรีย โดย ปกติในช่องปากของคนเราจะมีเชื้อแบคทีเรียอยู่เป็นจำนวนมากประมาณ 200-300 ชนิด เชื้อที่เป็นสาเหตุสำคัญที่ทำให้เกิดโรคอักเสบมักเป็นชนิดผสม ประกอบด้วยเชื้อสเตร็ปโตคอคคัส (Streptococcus mutans) เชื้อแบคทีเรียชนิด Treponema denticola และ Porphyromonas gingivalis โดย ที่ในน้ำลายของเรามีธาตุแคลเซียมและฟอสฟอรัสอยู่ เมื่อมีการสะสมเป็นเวลานานเข้าก็จะกลายเป็นหินปูนในปาก ถ้าทำความสะอาดฟันและช่องปากไม่ดีพอ แบคทีเรียในแผ่นคราบฟันจะไปยึดติดกับหินปูนเหล่านี้ และไปเกาะตามบริเวณขอบเหงือก ปล่อยสารพิษออกมาทำให้เหงือกอักเสบ

คราบหินปูน หินปูนหรือบางทีเรียกว่าหินน้ำลาย เกิดจากการที่แร่ธาตุในน้ำลายเข้าไปรวมตัวในคราบจุลินทรีย์จนเกิดเป็นคราบแข็ง และยึดเกาะติดกับเคลือบฟันแน่นยิ่งขึ้น หิน ปูนไม่สามารถกำจัดออกได้ด้วยการแปรงฟัน คราบหินปูนที่เกาะติดเคลือบฟันนั้น มีทั้งส่วนที่อยู่เหนือเหงือกและใต้ขอบเหงือก ส่งผลให้คราบจุลินทรีย์เพิ่มปริมาณการยึดเกาะกับเหงือกมากขึ้น ในระยะแรกเริ่มหินปูนจะมีสภาพนิ่มและต่อมาจะค่อยๆแข็งตัวขึ้น ความแข็งของหินปูนจะทิ่มตำเหงือก ทำให้เหงือกอักเสบ บวมและแดง

บุหรี่ จากรายงานการศึกษาทางระบาดวิทยา พบว่าผู้ที่สูบบุหรี่มีความเสี่ยงที่จะเป็นโรคเหงือกอักเสบมากกว่าผู้ที่ไม่สูบบุหรี่ 4-6 เท่า และพบว่าสารทาร์ในบุหรี่เป็นตัวเร่งการเกิดคราบหินปูนในผู้ป่วยที่เป็นโรคชนิดรุนแรงมากถึงร้อยละ 60

ปัจจัยทางพันธุกรรม พบว่าร้อยละ 30 ของผู้ป่วยโรคเหงือกอักเสบและโรคปริทันต์เกิดจากปัจจัยทางพันธุกรรม การ ศึกษาในระยะหลังพบว่ายีน interleukin-1 genotype เกี่ยวข้องกับการเกิดโรคเหงือกอักเสบที่รุนแรง โดยเฉพาะอย่างยิ่งในผู้ที่สูบบุหรี่

โรคเบาหวาน ผู้ป่วยโรคเบาหวานจะมีโอกาสเสี่ยงเป็นโรคเหงือกอักเสบรุนแรงและโรคปริทันต์มากกว่าคนทั่วไป และจะส่งผลให้การควบคุมระดับน้ำตาลในเลือดไม่ดีเท่าที่ควร โดยเฉพาะอย่างยิ่งในกรณีหญิงตั้งครรภ์ที่เป็นโรคเบาหวาน

ยาบางชนิด เช่น ยาเม็ดคุมกำเนิด ยาต้านอาการซึมเศร้า ยาระงับชัก และยาโรคหัวใจ

อนามัยช่องปาก การแปรงฟันไม่สะอาด หรือการที่ไม่ได้รับการขูดหินปูนนานๆ ครั้ง ตัวฟันจะถูกแผ่นคราบฟันเหนียวๆ หรือหินปูนเกาะติดอยู่ ใน แผ่นคราบฟันนี้จะมีอาหาร แบคทีเรียและสารพิษที่หลั่งจากแบคทีเรียผสมกันอยู่ ถ้าสิ่งเหล่านี้ยึดติดกับฟันเป็นเวลานานพอสมควร เหงือกก็มีอาการระคายเคือง

อาการ

โรคเหงือกอักเสบในระยะเริ่มแรกจะไม่มีอาการใดๆ เมื่อเป็นมากขึ้นอาการที่เริ่มแสดงออก คือเวลาแปรงฟันจะมีเลือดออกตามไรฟัน ซึ่งถ้าไม่ได้รับการรักษา อาการที่จะพบต่อมา คือ การมีกลิ่นปาก เหงือกจะมีสีแดงคล้ำ ปวดและเจ็บบริเวณเหงือก เหงือกจะเริ่มแยกตัวออกจากฟัน คนไข้จะรู้สึกว่าฟันของตนเองยาวขึ้น ฟันเริ่มโยกเพราะมีการละลายของกระดูกที่รองรับฟัน และเหงือกเป็นหนอง

เหงือกที่อักเสบจะมีลักษณะบวม แดง เป็นมัน ดูฉุๆ หาก ถูกขนแปรงตอน แปรงฟัน หรือลองใช้ไม้จิ้มฟันสอดเข้าไปในร่องเหงือก บริเวณดังกล่าวจะมีเลือดซึมออกมาได้ บริเวณ ที่เห็นได้ง่ายคือ บริเวณซอกฟัน ซึ่งเป็นบริเวณที่มักพบโรคก่อน และโดยที่มีเหงือกอักเสบ จึงมักจะเป็น บริเวณที่ถูกละเลยในการทำความสะอาด ซึ่งนานๆ เข้าจะเกิดหินปูน หรือหินน้ำลายร่วมด้วย มองเห็นเป็นแถบสีขาว ออกเหลือง แข็ง แปรงไม่ออก และหินปูนก็จะเป็นที่สะสมของเชื้อโรคด้วย จึงพบการอักเสบของเหงือกรุนแรงมากขึ้นได้

การมีเลือดออกจากเหงือกภายหลังการแปรงฟัน ทั้ง ที่แปรงสีฟันไม่ได้กระแทกเหงือกให้เกิดบาดแผลเลยนั้นน่าจะเป็นผลมาจากการ เป็นโรคเหงือกอักเสบ ซึ่งเกิดจากปฏิกิริยาตอบสนองของเหงือกต่อสารระคายเคืองจากคราบจุลินทรีย์ ผิวของเหงือกมีการเปลี่ยนแปลงเพิ่มการไหลเวียนของเลือดที่มาหล่อเลี้ยงหรือ มีเลือดคั่งอยู่ที่ผิดของเหงือกมากขึ้น ทำให้เหงือกบวมแดง เนื้อเยื่อที่ปกคลุมผิวเหงือกจึงบางขึ้นและฉีกขาดได้ง่าย ดังนั้นเมื่อได้รับการเสียดสีเพียงเล็กน้อยเช่นการแปรงฟันก็จะมีเลือดออก

การวินิจฉัย

โรคเหงือกอักเสบจะมีความผิดปกติเฉพาะบริเวณของเหงือกเท่านั้น ส่วนโรคปริทันต์นั้นเมื่อเป็นจะมีการ
ทำลายอวัยวะปริทันต์ที่อยู่รอบๆ ฟัน ได้แก่ เหงือก เยื่อยึดปริทันต์ กระดูกเบ้าฟัน และเคลือบรากฟัน ซึ่งเป็นอวัยวะที่ช่วยยึดให้ฟันอยู่ได้แน่นโดยไม่โยก ส่วนโรครำมะนาด หมายถึง โรคปริทันต์ในระยะลุกลามไปมากแล้วโดยมีอาการให้เห็นได้อย่างชัดเจน เหงือกบวมหรือมีหนองออกมาจากร่องของเหงือก ฟันมักจะโยก บางครั้งมีอาการปวด มักจะเป็นกับฟันทีละหลายๆ ซี่ในช่องปาก

การจะทราบว่าเราเป็นโรคเหงือกและ โรคปริทันต์หรือไม่ ให้หมั่นสังเกตและตรวจดูสุขภาพในช่องปากอย่างสม่ำเสมอด้วยตนเองตั้งแต่ยัง ไม่เป็นโรค เพื่อจะได้ทราบว่าในสภาวะปกติเหงือกและฟันของตนเองมีลักษณะเช่นไร วิธีง่ายๆ คือการอ้าปากส่องดูกับกระจกเงาหลังจากการแปรงฟันทุกวัน เมื่อเป็นโรคเหงือกและโรคปริทันต์ เราจะพบความผิดปกติที่เหงือกเป็นอันดับแรก เหงือกจะบวม มีเลือดออกง่าย สีเหงือกมักมีสีคล้ำไปจากเดิม เมื่อเป็นมากอาจมีหนองไหลออกข้างๆ ฟัน ตัวฟันแลดูยาวขึ้นเนื่องจากมีเหงือกร่น จะมีอาการปวดและฟันโยก ซึ่งหากพบรอยโรคในระยะแรกๆ สามารถรักษาเพื่อเก็บฟันไว้ใช้งานได้ต่อไป

การรักษา

อาการเหงือกบวมมีเลือดออก และบางครั้งมีหนองไหลออกมาจากร่องเหงือก เป็นอาการของโรค
ปริทันต์ในระยะที่ลุกลามมากแล้ว เมื่อตรวจดูจะพบว่า ร่องเหงือกมีความลึกมากขึ้น ซึ่งเรียกว่าร่องลึก
ปริทันต์ หรือบางทีก็เห็นตัวฟันยากเพิ่มขึ้นด้วย

การรักษาโรคเหงือกอักเสบและ โรคปริทันต์ขึ้นกับอวัยวะปริทันต์ที่อยู่รอบๆ ฟันที่ทำหน้าที่ช่วยยึดฟันให้แน่นว่าถูกทำลายไปมากน้อยแค่ไหน หากอวัยวะปริทันต์มีเหลือพอที่จะช่วยยึดฟัน อาจพิจารณารักษาโดยการทำความสะอาดร่องลึกปริทันต์ บางรายอาจทำการผ่าตัดเหงือกเพื่อลดความลึกของร่องลึกปริทันต์

mpacted teeth occur either because the path for their emergence is blocked, or because they have formed in the wrong position in the mouth. The eruption of teeth into mouth is not fully understood, but it is probably the result of rapid multiplication of the cells which from the tooth root, the pressure of which thrust the tooth through the overlying bone.

In cases of impaction, the problem is not usually one of failure within the developing tooth itself, but either of its position or of its relation to the other teeth nearby. Where there is crowding, that is, inadequate space in the mouth for all the teeth, the last teeth to erupt often have insufficient room, because the space available has all been taken by the teeth which have already emerged. Under these circumstances, the later erupting teeth may grow through, but out of line, or alternatively, they may be unable to emerge fully. When a tooth fails to erupt, either fully or partially, it is described as being impacted.

The cause of the crowding which leads to most impactions is not really understood, but it may arise as the result of the individual inheriting large teeth from one parent and small jaws from the other. It has also been suggested that there may be an evolutionary process in progress leading to a reduction in the size of the human jaw.

Sometimes, teeth are impacted as the result of their being formed in the in the wrong position. For example, upper canine teeth are sometimes too deeply placed, so that when they begin to erupt, they grow towards the alveolar bone [bordering the gum]. It is then not usually possible for them to erupt.

Impactions are comparatively uncommon in the milk teeth [called the deciduous dentition] because, as they are small, crowding is usually less. It is possible for any permanent tooth to become impacted, but impaction due to overcrowding most often affects wisdom teeth, the third molar, canines and premolars [situated in front of molar]. Impactions which are due to tooth development in incorrect positions affect upper canines more commonly than other teeth, and the problem often runs in families, showing that it inherited.

The decision of tooth extraction can be taken after special examination. It is a good idea to perform dental X-rays. Impacted tooth is more prone to cavities and decay. There is a great risk infection of surrounding tissue of wisdom tooth. The infection of surrounding tissue is known as periconitis. It is not possible to clean impacted tooth. Improper dental hygiene can cause tooth plaque.

A tooth can be removed with the help of a small operation. Operation can be conducted under local anesthesia. You should find a skilled dental surgeon. You have to take pain killers and antibiotics in the post-operative period. The recovery period is very short. Swelling is very common in post operative period. There are some complications of the surgery. But complications can be minimized if proper instructions are followed before and after the surgery.

The bottom teeth are used more for the grinding of food and the top front teeth are mainly used for biting.

Dental anatomy is a field of anatomy dedicated to the study of tooth structures. The development, appearance, and classification of teeth fall within its field of study, though dental occlusion, or contact among teeth, does not. Dental anatomy is also a taxonomical science as it is concerned with the naming of teeth and their structures. This information serves a practical purpose for dentists, enabling them to easily identify teeth and structures during treatment.

The anatomic crown of a tooth is the area covered in enamel above the cementoenamel junction (CEJ).^[3] The majority of the crown is composed of dentin with the pulp chamber in the center.^[4] The crown is within bone before eruption.^[5] After eruption, it is almost always visible. The anatomic root is found below the cementoenamel junction and is covered with cementum. As with the crown, dentin composes most of the root, which normally have pulp canals. A tooth may have multiple roots or just one root. Canines and most premolars, except for maxillary (upper) first premolars, usually have one root. Maxillary first premolars and mandibular molars usually have two roots. Maxillary molars usually have three roots. Additional roots are referred to as supernumerary roots.

Humans usually have 20 primary teeth (also called deciduous, baby, or milk teeth) and 32 permanent teeth. Among primary teeth, 10 are found in the (upper) maxilla and the other 10 in the (lower) mandible. Teeth are classified as incisors, canines, and molars. In the primary set of teeth, there are two types of incisors, centrals and laterals, and two types of molars, first and second. All primary teeth are replaced with permanent counterparts except for molars, which are replaced by permanent premolars. Among permanent teeth, 16 are found in the maxilla with the other 16 in the mandible. The maxillary teeth are the maxillary central incisor, maxillary lateral incisor, maxillary canine, maxillary first premolar, maxillary second premolar, maxillary first molar, maxillary second molar, and maxillary third molar. The mandibular teeth are the mandibular central incisor, mandibular lateral incisor, mandibular canine, mandibular first premolar, mandibular second premolar, mandibular first molar, mandibular second molar, and mandibular third molar. Third molars are commonly called "wisdom teeth" and may never erupt into the mouth or form at all. If any additional teeth form, for example, fourth and fifth molars, which are rare, they are referred to as supernumerary teeth.^[6]

Most teeth have identifiable features that distinguish them from others. There are several different notation systems to refer to a specific tooth. The three most commons systems are the FDI World Dental Federation notation, the universal numbering system, and Palmer notation method. The FDI system is used worldwide, and the universal is used widely in the United States.

Teeth (singular, tooth) are small white structures found in the jaws (or mouths) of many vertebrates that are used to tear, scrape, milk and chew food. Some animals, particularly carnivores, also use teeth for hunting or defense. The roots of teeth are covered by gums.

Teeth are among the most distinctive (and long-lasting) features of mammal species. Paleontologists use teeth to identify fossil species and determine their relationships. The shape of the animal's teeth are related to its diet. For example, plant matter is hard to digest, so herbivores have many molars for chewing. Carnivores, on the other hand, need canines to kill and tear meat.

Mammals are diphyodont, meaning that they develop two sets of teeth. In humans, the first set (the "baby," "milk," "primary" or "deciduous" set) normally starts to appear at about six months of age, although some babies are born with one or more visible teeth, known as Neonatal teeth. Normal tooth eruption at about six months is known as teething and can be painful.

Some animals develop only one set of teeth (monophyodont) while others develop many sets (polyphyodont). Sharks, for example, grow a new set of teeth every two weeks to replace worn teeth. Rodent incisors grow and wear away continually through gnawing, maintaining relatively constant length. Some rodent species, such as the sibling vole and the guinea pig, have continuously growing molars in addition to incisors.[1
